I hereby give permission to Hospice of San Joaquin to use my photographic image(s) for the Tots & Tail-Waggers photo contest and event advertisements. I further understand photos for the contest may be used by Hospice of San Joaquin for publication and may be used without further permission or credit.

If the contestant is under the age of 18, I the parent/legal guardian of the child, have read this release and approve the terms.
